Understand your health risks and earn $100

You spoke, we listened. Employees said they wanted the Wellness Incentive in their paycheck, so in 2014, active primary subscribers to UM medical plans could earn $100 by completing two simple steps by April 30, 2014: an online personal health assessment and health screening. Eligible employees who completed both requirements by April 30 earned $100 in their final May paycheck and were entered in a drawing for: 1 of 3 iPad Minis, a Samsung Galaxy Tab 3, or 1 of 2 FitBit Flexes.*

To make the process easier, we implemented a new, easier-to-use Cerner Health account that allows employees to track weight, exercise and food goals, participate in wellness workshops and connect with a dietician or trainer with the click of a mouse. All information is kept confidential; strict policies are followed to protect personal information.

Details on the two steps:

Step 1: Complete your Personal Health Assessment (PHA)
Create a Cerner Health Account, or log into an existing account by visiting https://www.healthyforlife.umsystem.edu. Employees were able to complete the PHA - a brief online questionnaire that helps participants understand health risks. Employees were provided with resources to help improve well-being.

Step 2: Complete your Health Screening
Choose ONE of these three ways to complete the health screening. Employees need only complete one health screening per year. Screening results had to be submitted by April 30, 2014.

  1. Visit your doctor. Employees could use results from an annual physical to complete the screening requirement. The form that had to be completed is located here. Employees were able to use results obtained from a doctor from May 1, 2013 through April 30, 2014.
  2. Schedule your FREE health screening on campus. Employees were able to schedule a health screening at a convenient time and location on campus via their Cerner Health Account. Visit www.wellness.umsystem.edu/2014screenings to view the 2014 screening schedule.
  3. Complete your Home Access kit by April 30. Home Access kits provided a method for completing a health screening at the employee's convenience in the privacy of their own home.
