Personal Health Assessment

What is the Personal Health Assessment?
The personal health assessment is an online questionnaire designed to identify where you are making healthy decisions and areas that you can target for improvement.

Why should I take it?
The personal health assessment helps you identify your health needs. Faculty and Staff who are subscribers to the UM medical plans can earn $100 in a health reimbursement account or health savings account simply by completing this 15-minute questionnaire and screenings.

It’s easy. Earn 100 points equal to dollars that increase your Health Reimbursement Account or Health Saving Account. Let’s get started! Here’s how.

Step 1: Create your Cerner Health account

Go to healthyforlife.umsystem.edu to create your Cerner Health account. Cerner Health is a personal health platform that allows you to maintain a personal health record. Using your account, you will also gain access to the improved Healthy for Life program.

Step 2: Join Healthy for Life

After creating your account, enter your employee ID and join the program.

Step 3: Complete your Personal Health Assessment and Health Screening

Your Personal Health Assessment is completed online in Cerner Health. It only takes about 15 minutes. Complete the program by scheduling and attending a brief Health Screening for a basic lab draw and a collection of other health measures. Once finished, you will have access to personalized recommendations to help you reach your health goals.

To receive the $100 Health Reimbursement Account or Health Savings Account credit, both the Personal Health Assessment and the Health Screening must be completed by April 30, 2012.
