BPM-703 Damage to University Property (Including Fire)

Reporting Damage to University Property

Written Report Required

All damage to, or destruction of, any University-owned or leased building or facility or its contents resulting from any cause including fire, must be reported in writing.

 
Definition of Contents

Includes: Any University equipment, machinery, furniture and fixtures which are essential to the operation or use of the building or facility.

 

Does Not Include:

Reporting Injuries

Written Report Required

Any personal injury sustained by any person in connection with the occurrence of the damage must be reported in writing.

REFERENCE: BP 704 Employee Injuries & Occupational Diseases and BP 707 Accidents - Students & General Public (Except Vehicle Accidents) identify the requirements for submitting personal injury reports.

Reporting Responsibilities

Damage reports must be:

  1. Initiated by the department of the individual who discovered the fire or damage and submitted to the following appropriate campus office within 24 hours after the damage occurs or is discovered:
    • UM - UM Risk & Insurance Management
    • Columbia - MU Business Services
    • Hospital & Clinics - UHC Patient Admissions
    • Kansas City - UMKC Business Services/Risk Management Coordinator
    • Rolla - UMR Environmental Health/Risk Management
    • St. Louis - UMSL Director of Business Services 
  2. Forwarded to the UM Office of Insurance and Risk Management by the campus representative within 48 hours after the damage occurs or is discovered.

 

Report Forms to be Used

All reports must be made on a Damage Report Form (UMUW Form 69).
