Reporting Accidents and Incidents

BPM-707 Students & General Public (Except Vehicle Accidents)

Definition of Accidents and Incidents (Students & General Public)

Accidents and incidents include:

 

Written Reports Required

All accidents or incidents involving students or the general public (except vehicle accidents) which result from, arise out of or directly relate to the University's premises (owned, rented or leased), University operations, or a University employee's position of employment, must be reported in writing.

 

Reporting Responsibilities

Accident and incident reports must be:

  1. Initiated by:
    • The academic staff member in charge of the student's activities at the time of the accident or incident or to whom the accident or incident was reported.
    • The person in charge of the building or facility or sponsoring the meeting or event attended by the student or member of the general public at the time of the accident or incident.
    • Any employee who witnesses an accident or to whom a threat or statement of intent to take legal action for personal injury is made.
  2. Submitted to the campus business office (UM - Office of Risk & Insurance Management) within 48 hours after the accident or incident.

 

Report Forms to be Used

All reports must be made on a Student or General Public Injury and Property Damage Report Form (UMUW Form 200).

 

Investigating Cause of Accident

The individual responsible for initiating the report should immediately try to determine if the accident was caused by an unsafe condition and if so, take the action necessary to prevent the occurrence of a similar accident.

 

Medical Attention for Injured Persons

Injured persons should be urged to seek medical attention:

 

NOTE: Employees should not make statements to the effect the University will be responsible for medical costs.
