Changes to Ride to Wellness and Rebate

Starting in November of 2017, you may submit your steps for points toward the 2018 Wellness Incentive.

Please note:

• For the month of October, prizes for MSPP will only be available as supplies last.
• Cateye Velo9 odometers will no longer be eligible for rebate if purchased after September 30, 2017.
• If you purchased a qualifying odometer prior to that date, you have until December 31, 2017, to submit your rebate request.

Participant Information

The following should be completed with the information of the individual who is claiming a rebate/prize under the Ride to Wellness Program. To claim a rebate/prize for another participant associated with the employee listed above, please submit additional request forms as needed.

Submit Miles and Claim Your Prize

This section is for claiming prizes only; the next section is for claiming rebates.

  • • Upload a screenshot or PDF of your mileage log showing each 500 milestone.

Please note that miles only need to be submitted upon reaching 500 miles, 1,000 miles, etc. as a rebate/prize is claimed; miles do not need to be submitted between each 500 miles. Screenshot images should be in jpg, gif, png, or pdf, and less than 400 KB.

Please see our current prize list for additional details on any of our prize options. You may select any prize listed for your current million milestone achievement or any previous milestone-i.e., If you claim a 1,000 mile prize you may choose the 500 mile prize or any prize in the 1,000-2,500 miles category.

In order to receive incentive points you will need to select '100 Wellness Incentive Points' as your prize.

* Effective October 1, 2014, and in accordance with IRS regulations, all awarded bookstore gift-card amounts must be reported as employee compensation and are subject to federal withholding and FICA taxes. Therefore, the gift-card amount awarded will appear on the employee’s payroll stub. If you choose not to receive the gift card, you can choose any other prize from the list.

Please provide a valid campus mailing address as this is the exact address that your prize will be mailed to. An invalid address may cause a delay in delivery or a lost package. If you do not receive campus mail, please provide a valid US postal service address.

Request a Rebate

The below questions MUST be completed in order to request a rebate. Incomplete rebate requests will not be processed. Please call 573-884-1312 if you require assistance or have lost your receipt, and Healthy for Life will assist you.

Upon riding 500 miles with a rebate eligible odometer, participants may claim one rebate of $20 through this program. Taxes and shipping fees are not rebate eligible. While participants may use any odometer, only the Cateye Velo9 purchased at a university bookstore is eligible for a rebate. Full eligibility requirements are available at http://umurl.us/RTW.
