Series Description:

This records series applies to electronic documentation in patient charts related to tests, treatments, and hospitalizations of all patients treated or for services performed at University Hospital and Clinics departments for adults, minors, students and deceased patients.  This retention also applies to the Schools of Psychology, Orthodontics, Ophthalmology, Dentistry, Vision, etc.

Series Cutoff: The last date of visit, treatment, test or therapy

Retention Period: 25 Years

Disposition Action: Securely Destroy or Delete

Some electronic record systems (such as Cerner) are not currently able to destroy electronic records because of system capabilities.  Those records will be considered permanent until the technology allows for automatic deletion at the end of the retention period.
