Series Description:

Registration materials (by participant) and individual enrollment applications for minors and youths (under the age of 18) authorizing participation in University programs such as 4-H, extension, field trips, sport camps, intercollegiate athletics, youth camps, high school and team camps, boot camps, robotics camps, etc.

Registration material may include, but are not limited to: applications, registrations, enrollment records, lists of attendees, medical treatment information, medical consents, concussion forms, correspondence about the youth participant, field trip permissions, activity records, release of liability waivers, and photo releases.    

Series Cutoff: Date of Enrollment

Retention Period: 20 Years

Disposition Action: Securely Destroy or Delete

Approval Date: 2023-09-30

Notes and Citations:

This retention authorization does not apply to the University Adventure Club records (Retention Authorization 37-422). 

This retention replaces Authorizations 00-13-026B, 77-125-05C and 37-402-01.  Send a copy of all Registration Form Templates to University Archives at the time of creation.  

Marketing Material - Youth program marketing material is covered under the Archival Review Authorization "Publications and Printed Material (ARC-017-01A)".
