The purpose of this page is to provide easy access to forms required for procedures related to academic grievances and faculty misconduct. The forms follow processes outlined in the Collected Rules and Regulations, Sections 330.110 (Standards of Faculty Conduct) and 370.010 (Academic Grievance Procedures).
Academic Grievance
The following forms are relevant for use as part of the Academic Grievance Procedures (CRR 370.010).
- Filing a Grievance
- Academic Grievance Respondent Rebuttal Statement: The Respondent should use this form to submit the required Respondent Rebuttal Statement. Such requests must be submitted within 15 calendar days from the date that they were provided with the original grievance filing form.
- Academic Grievance Respondent Rebuttal Statement Extension Request: As needed, the Respondent may use this form to submit a request to the Grievance Resolution Panel (GRP) for additional time to prepare a rebuttal.
- Responses to Grievance Resolution Panel (GRP) Recommendations
- Academic Grievance Appeal to GRP Findings Form: If needed, within 15 calendar days of the GRP rendering its findings and recommendations, either the Respondent or Grievant may use this form to submit an Academic Grievance Appeal to the Grievance Resolution Panel’s (GRP) findings and/or recommendations.
- Academic Grievance Findings Acceptance/Rejection Form: The Grievant should use this form to communicate acceptance or rejection of the Grievance Resolution Panel’s (GRP) findings and/or recommendations.
Faculty Misconduct
The following forms are relevant for use in cases related to alleged violations of the Standards of Faculty Conduct (CRR 330.110).
- Filing a Complaint
- Faculty Misconduct Filing Form: Use this form to report alleged inappropriate actions or incidents pursuant to CRR 330.110. Individuals eligible to submit a faculty misconduct complaint include University students, active full-time and part-time academic appointments, staff members, and administrators.
- Respondent Statement
- Faculty Misconduct Respondent Statement: The Respondent should use this form to submit a Respondent Statement within five (5) business days after receiving a Notice of Investigation.
- Appealing the Dean’s Decision
- Faculty Misconduct Respondent Request for Reconsideration of Dean’s Decision: If needed, the Respondent may use this form to request a reconsideration of the decision by the Dean to the Provost. Such requests must be submitted within five (5) business days of notification of the Dean’s decision.
- Faculty Misconduct Faculty Panel Response to Suspension: The Faculty Panel representative uses this form to submit, on behalf of the Faculty Panel Review Committee, a recommendation to the Provost stating the Panel’s agreement or disagreement with a suspension sanction. The Panel’s response is due within twenty (20) business days of notification of the Dean’s decision.
- Appealing the Provost’s Decision
- Faculty Misconduct Respondent Appeal to the Chancellor: If needed, use this form to submit an appeal to the Chancellor, appealing the Provost’s decision. Such appeals must be submitted within five (5) business days of notification of the Provost’s decision.
