These records are the medical records of students who receive medical treatment at Student Health Services or similar medical services provided to students by the University. The records may consist of, but not limited to, the following: admission records, medical histories and questionnaires, immunization records, x-rays, lab reports, medical treatments, correspondence, nurses notes, and any other information that may concern medical treatment.
Retain for ten (10) years after date of last treatment, then destroy.
Educational and reference materials are maintained for the benefit of students and staff. Materials may include, but not limited to, the following: newsletters, health pamphlets and booklets, informational materials on subjects such as, alcoholism, diets, sickle cell, venereal disease, diabetes, etc.
Retain for reference value until obsolete, then destroy.
This authorization supersedes records retention authorizations: 71-22, 75-28, 87-01 and 40-102.
